Lucas County Intersystem Cluster
Cluster is an entity mandated by the state to provide service coordination for eligible children and families within each county.

Purpose
The purpose of the Lucas County Intersystem Cluster is to work cooperatively to assure services are delivered in a timely and coordinated manner in the least restrictive environment possible on behalf of children and families whose service needs have been assessed as not being met by the existing linkages to agencies. This is accomplished by:
  1. Increasing the utilization of home-based and/or community-based services therefore reducing the incidence of long term, institutional placements.
  2. Efficiently using each system’s case management resources in the Cluster process.
  3. Increasing Cluster’s ability to plan, develop and put services in place in a timely manner.
  4. Favorably impacting service costs.
  5. Developing strategies to eliminate or reduce identified service and system barriers, or gaps in service.
Eligibility
Children are eligible for Lucas County Cluster services given the following criteria:
  1. Resident of Lucas County.
  2. Children to their 18th birthday, unless they are MR/DD eligible and then they may remain in Cluster until their 21st birthday. At these ages, they should be transitioned into the adult systems.
  3. Parent/ Guardian agrees to actively participate in the Cluster process and signs a Cluster participation agreement.
  4. Involvement in, or needing services from 2, or more, major public child serving agencies. The schools do not count as one of the agencies.
  5. Problems, issues, or needs in service coordination.
  6. Unmet needs due to service gaps, service barriers, or system barriers.
  7. A history of needs exceeding the services provided.
Cluster Service Team
The Cluster Services Team is made up of representatives of the major child serving agencies and is responsible for:
  • Developing and implementing joint service plans with the assistance of community service agencies
  • Reviewing each case on a routine (3 month) and emergency basis
  • Providing transition and exit case planning
  • Advocating for the provision of quality services and, as a team, providing cross system support for the creation and implementation of innovative service strategies.
